Registration Policies and Agreements

The following policies must be acknowledged and agreed to during the registration process. They are also kept here for your review and understanding before/after you register.

 

PAYMENT Agreement:

Payment is now required when you register. You will be taken to the Payment Page immediately after you submit this page. If you close your browser, or it times out after approximately 30 minutes of inactivity, and payment has not been received, the registration will be deleted and you will receive an email indicating that the registration was deleted due to lack of payment. You will need to re-register.

 

REFUND Agreement:

To be removed from a DHYS program once a player is registered for a season:

A) 100% of registration fees paid will be refunded if a player cannot be placed in a league, due to over-enrollment;

B) 100% of registration fees paid will be refunded if a player is withdrawn prior to the player draft AND your league is over-subscribed;

C) 100% of registration fees paid, with the exception of a non-refundable $25 fee to cover administrative costs, will be refunded if a player is withdrawn before player evaluations AND your league is not over-subscribed;

D) 50% of registration fees paid will be refunded if a player is withdrawn after evaluations but before the draft AND your league is not over-subscribed;

E) After the player draft, 100% of Registration Fees paid are non-refundable.

F) Other refunds may be given on a case by case basis at the Board's discretion with the exception of a flat $25 portion to be non-refundable to cover administrative costs.

 

CORONAVIRUS / COVID-19:

Assumption of Risk and Waiver of Liability Relating to Coronavirus/COVID-19

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. Like other viruses, COVID-19 is contagious, and is believed to spread mainly from person-to-person contact. As a result, federal and state health agencies, as well as federal, state and local governments relying upon the advice of those agencies, recommend social distancing and other preventative measures to mitigate the risk of the virus’s transmission. In many locations, this includes a prohibition against the congregation of groups of people above a certain number.

I acknowledge that Druid Hills Youth Sports (“DHYS”) has put in place reasonable measures based upon the aforementioned governmental guidance to reduce the spread of COVID-19; however, I understand and acknowledge that despite such measures being implemented and followed, DHYS cannot guarantee that I or my child(ren) will be free from exposure to  and infection from COVID-19 while attending and/or participating in DHYS activities. Further, I acknowledge and understand that attending and/or participating in DHYS events could increase my risk and my child(ren)’s risk of contracting COVID-19. 

I understand that I, my family (including my children), and others with whom we interact, may be exposed to additional health and safety risks related to COVID-19 as a result of our voluntary attendance at and/or participation in DHYS or DHYS-related activities and/or events.  I assume and accept these additional risks on my own behalf and on behalf of my family and child(ren), and agree to comply with instructions and guidance provided or recommended by DHYS and/or applicable federal, state, and local government requirements. I agree to follow best practices, including, without limitation, applicable hygiene and social distancing guidelines. Neither I nor my family (including my child(ren)) shall participate in or attend any DHYS or DHYS-related activities while any of us are experiencing or suspecting any potential symptoms of COVID-19, or have been in contact with anyone who has tested positive for COVID-19, and I agree  promptly to notify DHYS if I, my family or my child(ren) test(s) positive for COVID-19, or have/has been in contact with anyone who has tested or tests positive for COVID-19, and to thereafter absent ourselves from future activities until permitted by DHYS to resume participation/attendance. 

I expressly acknowledge and voluntarily assume the risk that I and/or my child(ren) may be exposed to or infected by COVID-19 as a result of attending or participating in DHYS events, and that such exposure or infection may result in personal injury, illness, permanent disability, and/or death. I understand that the risk of being exposed to or becoming infected by COVID-19 at or as a result of DHYS events may arise from the actions, omissions, or negligence of myself and others, including, but not limited to, DHYS volunteers, program participants and their families. 

I hereby voluntarily assume all of the foregoing risks and accept sole responsibility for any illness or injury suffered by me, my family or my child(ren) (including, but not limited to, personal injury, disability, and death), and resulting damage, loss, claim, liability, or expense, of any kind, that I, my family or child(ren) may sustain, experience or incur in connection with our attendance at or participation in DHYS events. On behalf of my family, including my children who may participate in DHYS events, I hereby release, discharge, hold harmless, and covenant not to sue DHYS employees, volunteers, agents, representatives, participants or spectators, from and against any and all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto.

I specifically understand and agree that this release and waiver of claims includes any claims arising from or based upon the actions and/or omissions of DHYS, its employees, volunteers, agents, and representatives, including those alleged to be the result of negligence, whether exposure to or infection with COVID-19 occurs before, during, after, or in any other manner related to, attendance of or participation in any DHYS or DHYS-related event, gathering, or activity.

 

MEDICAL RELEASE:

I, as parent or guardian of said player, hereby give approval for this player's participation in any and all DHYS league activities. In consideration for participation in DHYS league activities, I hereby grant permission to managing personnel or other league representatives to authorize and obtain medical care from any licensed physician, hospital, or medical clinic should the player become ill or injured while participating in league activities when neither parent nor guardian is available to grant authorization for emergency treatment. I hereby waive, release, absolve, indemnify and agree to hold harmless DHYS and its officers, directors, members, sponsors, supervisors, participants, volunteers and persons transporting the players to and from activities, for any claim arising from an injury to the player and do hereby waive any and all claims for the player, for myself, and/or for the players participation in DHYS league activities. I state that the player is in proper physical condition to participate in league activities. Although DHYS offers medical and dental accident insurance required by DeKalb County and other sanctioning organizations, I further understand that as a parent, I am responsible for any medical expenses incurred arising from an injury to the player and not covered by individual insurance or DHYS insurance. I have read, understand fully and agree to abide by the duties and responsibilities enumerated above. I also agree to abide by the by-laws of DHYS and DeKalb County rules and regulations governing public parks. I certify that the information given in this application is true and correct to the best of my knowledge and belief.

 

MEDIA AUTHORIZATION AND RELEASE:

Registration for participation in any DHYS activity constitutes an authorization and release on behalf of the registrant and any player(s) registered for DHYS to make full and complete use as DHYS deems appropriate of photographs, footage, video, live streaming, broadcasts or any similar use of the image, likeness, voice, and name ("licensed material")  of registrant and any player(s) registered individually and as part of groups when participating in any function or activity in any way associated with DHYS whether within the facilities of DHYS or otherwise.  Such authorization shall constitute a fully-paid, perpetual and royalty-free license to use the licensed material for commercial and noncommercial purposes as DHYS determines in its sole discretion and shall include, but not be limited to, the authority for DHYS and its agents to use, exploit, adapt, modify, reproduce, distribute, publicly display, and publicly perform the licensed material in any and all current or future forms or methods.

 

CODE OF CONDUCT Agreement:

Druid Hills Youth Sports believes that children deserve to have fun in a safe and caring environment. Parents are required to pledge to do everything you can to make this possible for all the children in our program.

I have read and agree to abide by the DHYS Code of Conduct posted at https://leagueathletics.com/Page.asp?n=36628&org=dhys.org. I also understand that failure to follow the Code of Conduct may result in my family's expulsion from the league.

 

BIRTH DATE Agreement:

I certify that my child's birth date on the registration form is correct. I understand that entering an incorrect birth date for any reason may result in my player's expulsion from the league and disqualification from any DHYS sponsored tournament or travel teams.

VOLUNTEER Agreement:

DHYS is volunteer driven and staffed. To keep costs down and continue our commitment to affordable programs, each family is expected to volunteer at least 3 hours of service over the course of the season. The most popular way to fulfill the volunteer requirement is by participating in Field Day.

My family will volunteer for at least 3 hours over the course of the season.
